Enter your Country of Birth and Country of Citizenship. Typically these are the same country. Neither of these should be the United States.
3. Enter Permanent Home Address
Your foreign permanent address in your home country is your permanent address in your home country of citizenship. This should not be a U.S. address.
4. Enter Dependents Information
You will be asked if you will be accompanied by dependents. They will require F-2 visa for their stay in the U.S. but they are not eligible to work.
5. Enter Your Financial Sponsor
You will be asked who is providing your financial support documents. Please enter either MPOWER Financing, PBC or Florence Technologies, PBC. Ask your nurse advisor which option you should choose.
